Coaching Agreement Highlights

• Coaching is not therapy, counseling, or medical treatment. It’s about mindset, performance, leadership, and confidence.

• You are responsible for applying what’s learned. Coaches cannot guarantee results.

• The initial commitment is 3 months minimum. After that, you can cancel with 1 month’s notice.

• Missed session refunds are only at Coach Jeff Becker’s discretion. You’ll get multiple reminders to schedule your sessions.

• Jeff Becker Inc. and its coaches are not liable for injuries, emotional stress, or other outcomes connected to coaching.

• Photos, videos, or testimonials may be used for educational/marketing purposes unless you opt out in writing.

This agreement remains valid forever—even if the client pauses or stops and later returns to work with Jeff Becker Inc. All videos, testimonials, recordings, quotes, results, and media captured during any coaching period may be used by Jeff Becker Inc. permanently for educational, promotional, and marketing purposes. This permission is irrevocable, royalty-free, and not withdrawn if services are canceled.

By signing below, you confirm that you have read and agree to the above summary and the full Coaching Services Agreement provided below.

Coaching Services Agreement

This Agreement is entered into by and between Jeff Becker Inc. (“Company”) and (Client Name Listed Above)(“Client”). Jeff Becker Inc. and its affiliated coaches (“Coach(es)”) agree to provide coaching services to Client under the following terms:

1) Coach–Client Relationship

A. Standards of Conduct: Coach(es) agree to maintain the highest ethics and professional standards.

B. Client Responsibility: Client is solely responsible for creating and implementing his/her own physical, mental, and emotional well-being, decisions, choices, actions, and results arising from the coaching relationship and interactions with Jeff Becker Inc. and its Coach(es). The Client agrees that neither Jeff Becker Inc. nor its Coach(es) shall be held liable or responsible for any actions or inactions, or for any direct or indirect results of coaching services.

C. Coaching Scope: Client understands that coaching is not therapy, counseling, or medical treatment and does not diagnose, prevent, cure, or treat any mental disorder or medical disease. Coaching may address areas including (but not limited to): performance, mindset, confidence, leadership, relationships, health, work, and education. Implementing choices and actions remains the Client’s sole responsibility.

D. Referrals: If the Client is under the care of a licensed professional (mental health, medical, legal, financial, etc.), it is the Client’s responsibility to inform that provider of the coaching relationship and seek professional guidance as needed.

E. Client Commitment: To enhance the coaching relationship, the Client agrees to communicate honestly, be open to feedback, and commit the necessary time and energy to fully participate.

2) Duration of Contract

The contract shall last a minimum of three (3) full calendar months.

Following the initial three (3) month period, the agreement will automatically continue on a month-to-month basis under the same terms unless otherwise communicated in writing by the Client or Client’s parent/guardian.

After the initial three months, either party may terminate the agreement with at least one (1) full calendar month’s written notice.

If the Client fails to provide proper notice, the Client may be charged for the upcoming billing cycle.

The contract may, in some circumstances, be terminated immediately with written permission from the Owner of Jeff Becker Inc.

Refunds Policy: Refunds for missed calls are at the sole discretion of Coach Jeff Becker. Clients will receive multiple weekly reminders to schedule their upcoming sessions.

3) Limited Liability

Except as expressly provided in this Agreement, Jeff Becker Inc. makes no guarantees or warranties of any kind, express or implied, regarding the coaching services.

In no event shall Jeff Becker Inc. or its Coach(es) be liable for any indirect, consequential, or special damages. The Client’s exclusive remedy, and Jeff Becker Inc.’s entire liability under this Agreement, is limited to the total amount paid by the Client to Jeff Becker Inc. for services rendered up to and including the termination date.

4) Release of Liability

By signing this Agreement, the Client (and/or the Client’s parent/guardian if under 18):

• Acknowledges that participation in coaching sessions may involve discussions, exercises, or activities that could result in physical, emotional, or mental discomfort.

• Voluntarily assumes all risks associated with participation in coaching services provided by Jeff Becker Inc. and its Coach(es).

• Releases, waives, and forever discharges Jeff Becker Inc., its owners, employees, contractors, and affiliated Coach(es) from any and all claims, demands, or causes of action, whether known or unknown, arising out of or connected with participation in coaching services, including but not limited to claims alleging negligence.

• Affirms that this release is binding upon the Client and their heirs, legal representatives, and assigns.

5) Photo/Media Release

By signing this Agreement, the Client (and/or the Client’s parent/guardian if under 18):

• Grants Jeff Becker Inc. and its Coach(es) permission to record, photograph, or otherwise capture images, video, and/or audio of coaching sessions, team activities, or related events.

• Authorizes Jeff Becker Inc. to use such media for educational, promotional, and marketing purposes (including but not limited to social media, websites, newsletters, and training materials).

• Understands that no compensation will be provided for the use of such media.

• May request in writing, at any time, to opt out of future use of their individual image or likeness, which Jeff Becker Inc. will honor on a going-forward basis.

  
  

6) Entire Agreement

This document reflects the complete agreement between Jeff Becker Inc. and the Client, and supersedes all prior written or oral agreements. Amendments must be in writing and signed by both parties.

7) Dispute Resolution

If a dispute arises that cannot be resolved by mutual agreement, the Client and Jeff Becker Inc. agree to attempt good-faith mediation for up to 30 days. If unresolved, and legal action is pursued, the prevailing party shall be entitled to recover attorney’s fees and court costs.

8) Severability

If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full effect. Any unenforceable provision will be modified to the minimum extent necessary to make it valid and enforceable.

9) Waiver

Failure by either party to enforce any provision of this Agreement does not constitute a waiver of that party’s rights to enforce it in the future.

10) Binding Effect

This Agreement shall be binding upon the Client, Jeff Becker Inc., its affiliated Coach(es), and their respective successors and permissible assigns.

11) Survival, Perpetuity & Media License

The Client agrees that all provisions of this Agreement relating to liability release, intellectual property, confidentiality, non-disparagement, and media usage shall survive the termination, cancellation, or expiration of coaching services and remain in full force and effect indefinitely.

The Client grants Jeff Becker Inc. and its affiliated Coach(es) an irrevocable, worldwide, perpetual, royalty-free license to record, reproduce, publish, distribute, display, and otherwise use the Client’s name, image, likeness, voice, performance data, testimonials, and any media captured during coaching sessions or related events, in any format now known or later developed, for educational, promotional, commercial, and marketing purposes.

This license shall not be revoked due to cancellation of services or at any future date, and remains in effect whether the Client is actively participating in services or not. The Client acknowledges that this permission is binding upon their heirs, legal representatives, and assigns.

AI-Assisted Session Documentation & Transcription Consent

I understand and acknowledge that Jeff Becker INC. and it's affliated Coach(es) may use approved technology, including Zoom AI Companion or similar AI-enabled transcription and meeting-summary tools, during virtual coaching sessions for the purpose of documenting coaching conversations, creating session summaries, identifying action items and next steps, maintaining continuity between sessions, and preparing future coaching materials.

These technologies may process spoken content from the coaching session and may generate a transcript, summary, notes, or other written documentation. Jeff Becker INC. may use approved third-party technology providers, including Zoom, to process this information.

Session summaries and related coaching notes may be reviewed by authorized Jeff Becker INC. personnel and maintained as part of the athlete/client’s coaching history. For minor athletes, appropriate portions of the session summary may also be used to prepare routine parent/guardian progress updates consistent with JB MPC’s confidentiality and athlete-privacy practices.

I understand that AI-generated summaries may contain errors or incomplete information and are used as an administrative and coaching-support tool rather than as a verbatim or definitive record of the session.

JB MPC will not use private coaching-session content for advertising, social media, testimonials, or other public marketing purposes without separate appropriate permission.

By checking the box below, I consent to the use of AI-assisted transcription, processing, summarization, and documentation as described above.

Acknowledgment & Consent: By signing, Client confirms that:

• They have read and understood this Agreement.

• They consent to receive coaching services.

• If the Client is a minor, the parent/guardian has full legal authority to consent to these services.

• They consent to the Photo/Media Release unless otherwise notified in writing.

• They acknowledge receipt or access to a copy of this Agreement.
